To create the paste, Miller and his team harvest the bone marrow from patients at the time of surgery with a small needle aspiration of bone marrow from the iliac crest. The marrow is spinned down and mixed with particulate collagen/glycosaminoglycan.
Not a lot of stem cells are harvested in this manner, according to recent articles, averages 30K cells. This is in contrast to other expansion type techniques that yields millions of cells. Microfracture technique is probably a good way to go, however using a scaffold or paste is not required, as many investigators have previously demonstrated. All is required is large numbers of stem cells, that normally number 10-100 cells per million bone marrow cells, and some kind of carrier, like hyaluronic acid, as many authors have demonstrated. At the Dubai Bone and Joint Center we are on the edge of having a program that will regenerate near normal articular cartilage in both architecture as well as type 2 collagen content. We are starting to move in the right direction!!!
